Fingerprint Clearance Cards (IVP and Non-IVP)

 What to Bring to
Your Appointment

Walk in prepared. Here is what you will need:

    • A valid government-issued photo ID — driver’s license, state ID, U.S. passport, military ID, or employment authorization card

  • Any instructions or paperwork provided by the agency requiring your fingerprints 

  • If applying for a Fingerprint Clearance Card: a cashier’s check or money order payable to 
    “Arizona Department of Public Safety” (if required by your agency)

Requirements may vary depending on your specific agency or licensing board.
